Shem Creek Health Center, located on the twenty-four-acre campus of South Bay at Mt. Pleasant, offers a full range of health care services to meet the needs of the individual. Offering in-patient and out-patient rehabilitation, assisted living, memory care, and skilled nursing care, the staff of Shem Creek provides services for not only the residents of South Bay but also the surrounding Mount Pleasant area.

Memory Care
Shem Creek Health Center offers spacious accommodations in our Memory Care area for those experiencing cognitive impairment due to dementia, including Alzheimer’s care, as well as other cognitive impairments. A robust calendar of activities based on each resident’s assessment focuses on maintaining memory and stimulating socialization. Team members serving in Shem Creek’s Memory Care area receive specialized training focusing on enhancing the quality of life of cognitively impaired residents while providing a safe environment.

Respite Care
Respite care is available for those who need support on a more temporary basis. The responsibility of caregiving can be overwhelming, and a respite stay at our community can provide much needed relief. Respite can also be used is a caregiver goes on an extended vacation, ensuring the resident is well-cared for in their absence.
Respite care service are available for a variety of care needs, and can be a great option after a hospitalization or rehab stay where a resident might not be ready to return to an environment without services.
Not sure if a residential assisted living is right for you or your loved-on? Respite can also serve as a trial period, allowing you or your loved one to immerse themselves in the lifestyle that Shem Creek Health Center provides.

Short-Term Rehabilitation
When a senior undergoes elective surgery or an unplanned, acute hospital stay or surgery there is often a need for Licensed Therapy services provided in Skilled Nursing. Shem Creek Health Center offers Physical, Occupational and Speech therapies to assist short-term residents to return to their prior living arrangements and lifestyle. Our state of the art Rehab Gym offers a wide array of modalities to accelerate your recovery and the therapists receive ongoing training to ensure we are providing the highest level of Skilled Therapy Services available.
The Wellness program at South Bay will have an open referral system and an open line of communication between the Rehab department and the Wellness department to ensure a full continuum of care. If a resident is in need of rehabilitation services, once they are discharged and graduate from therapy, they will have a smooth, seamless transition into the Wellness Center and will be properly oriented to the staff and programming to continue their plans of care and take their bodies to the next level and beyond.

Making the decision to have elective surgery takes preparation – from choosing a surgeon to identifying what services are needed following the procedure. Our Pre-Registration Program makes it easy to plan the next steps for recovery following an elective surgery. Our Admissions Office requests a surgery date, insurance information, completes the admission paperwork and confirms your room reservation.

Skilled Nursing
When there is a recognized need for care to be provided on a continuous basis by licensed medical staff, Shem Creek offers spacious private apartments with private bathrooms within our Skilled Nursing accommodations. Our Skilled Nursing team members receive specialized and on-going training on the compassionate and professional care we provide for our residents.

Residents will find thorough, accessible, and complete medical care for acute illnesses, chronic diseases, and preventive care, as well as access to MUSC specialists, as needed.

The Medical University of South Carolina (MUSC) Health’s Geriatrics program provides total medical support for our Skilled Nursing and Short-term Rehabilitation residents in Shem Creek Nursing and Rehab as well as the full spectrum of Primary care medical services for our Assisted Living residents in Shem Creek Health Center. Additionally, our Independent Living residents of South Bay at Mount Pleasant also have opportunity to have their Primary Care needs addressed through the on-campus clinic once they are established with MUSC Health’s Geriatrics program.


Shem Creek Health Center at South Bay is also serving as a clinical site for MUSC’s new Geriatric Medicine Fellowship program, a foundational building block for the institution’s plan to grow and develop a sustainable geriatrics workforce.

Dr. Haley
Dr. Sean P. Haley, M.D. MPH, is a family medicine physician specializing in full-spectrum care and public health. He earned his Bachelors of Science in Biology from Syracuse University, a Master’s degree in Public Health from Syracuse University and SUNY Upstate Medical University, and a Doctorate in Medicine from SUNY Upstate Medical University where he was inducted into the Gold Humanism Society. Dr. Haley completed residency training in family medicine at Trident/MUSC Family Medicine Residency in Charleston and served as Chief Resident. His clinical interest areas include preventive medicine, chronic disease management, community medicine, population health, public health, and global and under-served care. He has extensive experience working in the not-for-profit sphere within community development and with the resettled refugee population. He is an active supporter of Young Life in Charleston.
